If you know of an applicant that requires Spanish language forms, please direct the applicant to the Diocesan website where the Screening Form, Acknowledgement Form, and Program Booklet are all available in Spanish. These can be downloaded and completed by the applicant. (A Spanish language Reference Form is not available. Applicant is encouraged to list references that we may contact by phone.)
